New A&S Personnel Forms

To request email, server, or facilities access for an employee, you will now need to complete two forms — the A&S Building and Classroom Access Request and the A&S Email and Server Access Request.

Use the A&S Building and Classroom Access Request to

  • Report a new employee in your unit so that eTech can ensure he/she has building and classroom access
  • Report the departure of an employee (one who has already left or is about to leave) so eTech can cancel his/her building and classroom access
  • Request changes to building and classroom access for a current employee

Use the A&S Email and Server Access Request to

  • Report a new employee in your unit so that eTech can ensure he/she has email and server access
  • Report the departure of an employee (one who has already left or is about to leave) so eTech can turn off his/her email and server
  • Request changes to server access for a current employee

You must complete the HR New Faculty/Staff Form before submitting an A&S access request.
